    Primarily there are three degrees of nerve injury: Neurapraxia is often the result of a compressive or crush injury to the nerve where conduction is blocked due to myelin damage, resulting in a focal conduction block without Wallerian de-generation as the axon itself remains intact with recovery generally evident in three to six weeks when myelin continuity is restored. The complete interruption of axons is termed axonotmesis. Neurotmesis is the complete physiologic transection of axons as well as all supporting tissue. Neurotmesis results in changes within the nerve cell body and regeneration with a variable recovery course.

    After nerve injury, muscle atrophy occurs at the motor endplate, followed by fibrosis. If reinnervation does occur, both old and new motor end plates will be activated. Although the muscle is atrophied and fibrotic, it can remain viable for up to eighteen months; however, if reinnervation does not occur within one year, the chance of functional recovery diminishes. Patient age is the most important factor to consider in evaluating the success of nerve recovery following repair while the level of injury also plays a role, with distal injuries having better recovery results than proximal injuries. Neuromodulation and Neurostimulation are novel techniques used for nerve repair and regeneration, as opposed to costly surgeries.

    Both, neuromodulation and neurostimulation encompasses the application of targeted electrical, chemical and biological technologies to the nervous system in order to improve function and quality of life. The appropriate therapy (low level electrical pulses or micro-doses of medicine) are targeted to nerves along the spinal cord to block pain signals to the brain. It alleviates or lessens pain without putting patients into excessive sedation. It is an FDA approved procedure, wherein patients can also remove implants. However, currently neuromodulation does not guarantee reimbursement coverage.

    More importantly, existing and emerging neuromodulation treatments also include application in medication-resistant epilepsy, chronic head pain conditions, and functional therapy ranging from bladder and bowel or respiratory control to improvement of sensory deficits. Technical improvements include a trend toward minimally invasive (or noninvasive) systems; as well as smaller, more sophisticated devices that may have automated feedback control, and conditional compatibility with magnetic resonance imaging. It has been investigated for other chronic conditions, such as Alzheimer's disease, depression, chronic pain, and as an adjunctive treatment in recovery from stroke.
